Before this project, patient intake and assignment were being managed in Excel spreadsheets. This created inefficiency, duplication, and risk. Care managers had no consolidated view of patient status, and supervisors struggled to see provider capacity. The lack of visibility made it difficult to prioritize patients, balance workloads, and ensure evaluations were completed within required timeframes.

We replaced fragmented spreadsheets with a role-centered dashboard that delivered clarity and actionability. We initially tried using patient cards for sorting and filtering, but went back to a traditional table view that was more familiar for users

A sortable, searchable table that provides visibility into intake status, pending evaluations, and patient details.
Patient Detail View
Quick access to case summaries and clinical forms, with tools for notes, assignments, and workflow shortcuts.
Caseload Assignment View
Provider caseloads displayed at a glance, helping supervisors assign patients without creating overload.
Outcomes
The new dashboard replaced the old Excel-based process with a scalable, role-centered tool. It provided:
