Care Management Dashboard

Project Overview

The Transitional Care Management (TCM) team needed a more effective way to manage patients during the intake and enrollment process. The goal was to provide a single, consolidated dashboard that would allow care managers to:

 

  • Identify unassigned patients
  • Track patients pending 24-hour evaluation
  • View consolidated clinical information
  • Quickly launch relevant clinical forms
  • Monitor provider caseload capacity
  • Assign patients without overloading resources

The Challenge

Before this project, patient intake and assignment were being managed in Excel spreadsheets. This created inefficiency, duplication, and risk. Care managers had no consolidated view of patient status, and supervisors struggled to see provider capacity. The lack of visibility made it difficult to prioritize patients, balance workloads, and ensure evaluations were completed within required timeframes.

Design Approach

We replaced fragmented spreadsheets with a role-centered dashboard that delivered clarity and actionability. We initially tried using patient cards for sorting and filtering, but went back to a traditional table view that was more familiar for users

Outcomes

The new dashboard replaced the old Excel-based process with a scalable, role-centered tool. It provided:

  • A single source of truth for patient intake and assignments

  • Improved speed and accuracy of patient assignment

  • Reduced risk of missed 24-hour evaluations

  • Clear visibility into provider capacity to balance workloads

 

The dashboard is now a daily workflow essential for the TCM team, supporting both frontline care managers and supervisors.
